Files documenting this federally funded project to develop a program to lower morbidity and mortality for diabetes mellitus by improving coordination of services and utilization of resources, and making increased use of information describing the results of care. The project was part of the Community Diabetes Control Demonstration project, begun under the National Diabetes Mellitus Research and Education Act, 1974 [Public Law 93-354], and administered nationally through the National Center for Disease Control.
